KC's Fitness Sunday Sparring Liability Waiver

Full Name

In consideration for participating in Sunday Sparring at KC’s Fitness Inc., I do hereby waive, release, and forever discharge Kevin Cunningham, KC’s Fitness Inc., as well as any of his employees or independent contractors, from any and all responsibility or liability for injuries or damages resulting from my participation.

I understand that boxing sparring is a contact sport where strength, flexibility, and aerobic exercise, including the use of equipment for boxing sparring, involves risk of injury and even death, and that I am voluntarily participating in these activities and using sparring equipment with knowledge of the risks involved. I hereby agree to expressly assume and accept any and all risks of injury or death.

I acknowledge that I have either had a physical examination and have been given my physicians permission to participate in Sunday Sparring, or that I have decided to participate in boxing sparring at KC’s Fitness, Inc., without the approval of my physician and do hereby assume full responsibility from my participation in such activities, as well as in the utilization of any and all sparring equipment necessary for boxing sparring.

I do hereby further declare myself to be physically sound and suffering from no condition, impairment, disease, infirmity, or other illness that could limit or prevent my participation in boxing sparring or use of sparring equipment except as otherwise submitted in writing to Kevin Cunningham, owner of KC’s Fitness Inc.

I consent to the use of photo/video taken at KC's Fitness for use in marketing, social media, and in other communications related to the mission of KC's Fitness Inc.

I declare that the information provided in this Sunday Sparring liability waiver is accurate and complete.
