KC's Fitness Liability Waiver

Full Name

In consideration for participating in any/all activities at KC's Fitness Inc., I do hereby waive, release, and forever discharge Kevin Cunningham, KC's Fitness, Inc., as well as any of his employees or independent contractors, from any and all responsibilities or liability for injuries or damages resulting from my participation.

I understand that strength, flexibility, and aerobic exercise, including the use of boxingand/or weightlifting equipment, involvesa risk of injury and even death, and that I am vountarily participating in these activities and using equipment with knowledge of the risks involved. I hereby agree to expressly assume and accept any and all risks of injury or death.

I acknowledge that I have either had a physical examination and been given my physician's permission to participate, or that I have decided to participate in activities at KC's Fitness, Inc. without the approval of my physician and do hereby assume all responsibility for my participation in activities, as well as in the utilization of any and all exercise and martial arts equipment.

I do hereby further declare myself to be physically sound and suffering from no condition, impairment, disease, infirmity, or other illness that could limit or prevent my participation or us of equipment except as otherwise submitted in writing to Kevin Cunningham, owner of KC's Fitness, Inc.

I consent to the use of photo/video taken at KC's Fitness for use in marketing, social media, and in other communications related to the mission of KC's Fitness Inc.

I declare that the information I’ve provided is accurate & complete.
